In this paper we describe the process by which we identified a comprehensive list of flight deck automation issues; compiled evidence related to those issues from accident reports, incident report studies, experiments, surveys, and other studies; and disseminated our findings to the aviation community. In the paper we present examples of the issues, examples of evidence, a summary of our results, and the means by which users can access our complete findings.
