Very few travellers will seek the advice of a travel medicine specialist to understand if they are ‘fit to travel’. Fitness to travel is an abstract concept that is greatly influenced by the predisposition of the traveller. It requires the travel medicine practitioner to have a good knowledge of travel trends and the risks and efficacy of preventive measures to ensure the health of a traveller. Travel health practitioners should find the right balance between unduly scaring travellers and informing them of the appropriate risks. This chapter highlights specific issues that could increase the risk of morbidity and mortality to travellers. It summarises special considerations for some types of travellers, including allergic travellers, diabetic travellers, travellers with cardiorespiratory problems, and pregnant women. The chapter also holds forth on the elderly traveller, the epileptic traveller, travellers with psychiatric conditions, and the immunocompromised traveller. Environmental risks during travel are briefly discussed.
