The causes of the disastrous explosion on the Piper Alpha North Sea oil platform are discussed. They include inadequate training, determination to avoid down-time, and the unauthorised recommissioning of incompletely repaired equipment. The article then considers SCADA systems for use on platforms, incorporating subsystems relating to permission to work, gas lift optimisation, reporting and archiving, pipeline integrity, and well testing and fiscal metering.
