A study for developing a methodology for prediction of the lives of avionic printed wiring board assemblies was completed. The first task for this study was the identification of latent defects and failure mechanisms that are major contributors to avionic failures. This paper presents the findings of this part of the study. The major locations and types of defects causing avionics field failures were identified by an extensive examination of data on failures in Hughes Aircraft Company and other avionic systems, industrial and Air Force Reliability Analysis Center defect data and Hughes Failure Analysis Reports; and by collecting the experience of experts. Over 150 reports were reviewed. A systematic procedure was followed to separate facts from fiction. The fiction part lies in semantics. Often times a removal of an assembly is called a failure even if the assembly was found to be good under more detailed testing. Failure data, over 11000 out of the 23000 available failure reports, from four major avionic systems manufactured by Hughes were also reviewed along with selected failure analysis results containing information from scanning electronmicroscope investigations.
