During a routing flight training mission a UH-58 helicopter broke in half and crashed into a lake. The cause of this accident was the pilot who liked to fly fast at a very low altitude and did not abide by the flying standards. Everyone in his unit knew of his unsafe flying but did not about it. The lessons to be learned from this accident are when someone does not follow the flying standards or refuses to fly by the rules and takes unnecessary risks, someone has to care enough to do something about it and share the responsibility when something happens.
