One vehicle, a 1979 Ford van, because of the condition of the bridge, became airborne, struck one of the falling bridge spans, and entered the water. The one occupant exited the van, swam to shore before the van sank in 20 feet of water, and sustained minor injuries in the accident. The National Transportation Safety Board determines that the probable cause of the collapse was the undetected deterioration of the steel H-piles due to the inadequate inspection of the underwater bridge elements by the State of Alabama.
