At 11:25 a.m. on December 17, 1991, National Railroad Passenger Corporation (Amtrak) train 87, operating on CSX Transportation Inc. (CSXT) track, derailed in Palatka, Florida. As a result of the investigation of the accident, the National Transportation Safety Board identified three major safety issues: the inattentiveness of the engineer and fireman; use of prescription and over-the-counter drugs by operating crewmembers; and the adequacy of Federal Railroad Administration (FRA) regulations for inspecting modern passenger car brakes. Following a brief accident narrative, the report discusses these issues, as well as the effectiveness of Amtrak's equipment inspection and maintenance programs and the securement of food service appliances and seats in passenger cars.
