About 0805 Atlantic standard time on August 2, 1984, Vieques Air Link, Inc., Flight 901A, a Britten-Norman BN-2A-6 Islander, crashed into the ocean shortly after takeoff from Vieques, Puerto Rico. The pilot and his eight passengers were killed, and the airplane was destroyed on impact with the water. The investigation revealed that the left engine lost power shortly after takeoff and that the pilot lost control of the airplane. The National Transportation Safety Board determines that the probable cause of the accident was the failure of the pilot to execute the emergency engine-out procedure properly shortly after takeoff.
