On August 30, 1991, eastbound Burlington Northern Railroad freight train 602 collided head on with westbound Burlington Northern Railroad freight train 603 at milepost 85.55 north of Ledger, Montana. Three crewmen were killed, and four were severely injured. The safety issues discussed in the report include track warrant delivery and radio procedure, recurrent/refresher dispatcher training, management oversight and dispatcher support, locomotive radio testing, and positive train separation.
