In response to your request, this report discusses the mechanisms that National Aeronautics and Space Administration (NASA) has in place capture, disseminate, and apply past lessons learned towards mission success. We found that NASA's processes, procedures, systems do not effectively capture and share lessons learned therefore, NASA has no assurance that lessons are being applied future missions. We include recommendations to the NASA on ways to strengthen the agency's lessons learning processes systems.
