The report explains the capsizing and sinking of the U.S. self-elevating liftboat TITAN in the Gulf of Mexico on June 29, 1989. The safety issues discussed in the report are the shipboard emergency alerting systems; high-water alarms in floodable compartments; lifefloat deployment; liftboat safety systems; stability information; liftboat leg fractures; welding procedures; and regulations and inspection procedures. Safety recommendations addressing these issues were made to the U.S. Coast Guard; Coastal Marine Lift Barges, Inc.; Laredo Construction, Inc.; and Offshore Marine Services Association.
