The report describes an inflight collision, in which the passengers and crewmembers were fatally injured and both aircraft were destroyed. The National Transportation Safety Board determines that the probable cause of the accident was the failure of both flightcrews to detect visually the other aircraft in sufficient time to initiate evasive action.
