Provided are: a method for preparing a modified conjugated diene polymer; a modified conjugated diene polymer prepared thereby which is excellent in terms of compatibility with an inorganic filler in the rubber composition and has a high modification ratio; and a rubber composition including the same which has excellent physical properties including abrasion resistance, etc., and machinability. The preparation method of the present invention comprises the following steps: polymerizing a conjugated diene compound by using a catalyst for polymerization including a neodymium compound represented by chemical formula 1, an alkylating agent, and a halogen compound in the presence of a polymerization solvent, to form a conjugated diene polymer having an active organometallic portion; and making the conjugated diene polymer having the active organometallic portion react with a modifier represented by chemical formula 2. In the chemical formulas 1 and 2, Cy, R_a to R_c, R^1 to R^4, and m are defined as described in the specification.
