The invention relates to a hydraulic shock absorber, a suspension system and a vehicle, the hydraulic shock absorber comprises an inner cylinder, the inner cylinder is provided with a piston rod, the space between the inner cylinder and the piston rod is filled with hydraulic oil, the outer wall of one end of the inner cylinder is coaxially sleeved with a first outer cylinder, and a first cavity is formed between the inner cylinder and the first outer cylinder; the piston rod penetrates out of the first outer cylinder, the outer wall of the other end of the inner cylinder is coaxially sleeved with a second outer cylinder, and a second cavity is formed between the inner cylinder and the second outer cylinder; the oil guide and heat dissipation assembly is communicated with the first chamber and the second chamber; the three-way valve is installed in the second cavity, an oil inlet and an oil outlet of the three-way valve are communicated with the inner barrel, a one-way oil outlet of the three-way valve is communicated with the second cavity, and a one-way oil inlet of the three-way valve is communicated with the first cavity through an oil conveying pipeline. The hydraulic oil dissipates heat to the external environment in the flowing process, and the hydraulic oil is prevented from losing efficacy due to high temperature.
