The invention belongs to the technical field of steel rail fatigue tests, and particularly discloses a steel rail fatigue test device and a steel rail fatigue test method. The steel rail fatigue test device comprises a first supporting roller and a second supporting roller which are used for supporting a steel rail; the vertical loading clamp is used for applying a vertical load to the steel rail and comprises a rolling pressure head capable of rolling on a rail head of the steel rail; the liquid storage device is used for storing a lubricating medium; and the spray head is in fluid communication with the liquid storage device and is used for applying the lubricating medium to a rail head of the steel rail. The steel rail fatigue test device provided by the invention can accurately simulate the rolling loading service state of the wheel with the lubrication condition in the fatigue test process, and is beneficial to more accurately evaluating the steel rail service performance under the rail head lubrication condition.
