An INS and transponder combined train operation monitoring device positioning system comprises an INS inertial navigation system, a train operation monitoring host, a transponder and a ground information receiving unit, the transponder is arranged at a mileage point, a forecast point, an entrance point and an exit point of a ground track, geodetic coordinates of the transponder are stored in the train operation monitoring host; the ground information receiving unit is used for receiving a transponder signal; the train operation monitoring host is used for acquiring geodetic coordinates of corresponding transponders based on transponder signals; the INS inertial navigation system is used for starting to work when the initial reference coordinate is obtained, and calculating the geodetic coordinate in the train operation process on the basis of the reference coordinate position; and the train operation monitoring host is also used for updating the reference coordinates of the INS inertial navigation system based on the obtained geodetic coordinates of other transponders so as to carry out position error correction in the train operation process.
