The invention relates to the field of balance crane equipment, and discloses a pneumatic cylinder mechanism of a pneumatic balance crane. The pneumatic cylinder mechanism of the pneumatic balance crane comprises a balance pneumatic cylinder, supporting side plates, a ball screw, piston plates and a cable drum, wherein two sides of the balance pneumatic cylinder are both provided with the supporting side plates; the ball screw is arranged between the two supporting side plates; and the ball screw is provided with the piston plates and the cable drum. According to the pneumatic cylinder mechanism of the pneumatic balance crane, the design of the supporting side plates on two sides of the cable drum is in cooperation with the use of a sealing separation plate, a first sealing plate, a secondsealing plate, first fixing shafts, second fixing shafts, a movable sealing plate and a sealing plate; when the balance crane moves up without load, a negative pressure is formed between the first sealing plate and the piston plate positioned on the right side of the cable drum, and the negative pressure increases as the cable drum continues to move so as to limit the leftward movement of the cable drum; and therefore, the balance crane gradually tends to be in a stable state when moving up without load so that the impact degree on the balance crane is relatively low and the service life of the balance crane is prolonged.
