From 20 to 50% of rocket test failures may be directly attributed to human error; findings based on Titan first and second stages reveal classifiable human failures in installation of components or omissions, improper use of switches, faulty settings and wiring, and improper checkout, fueling without adequate decontamination procedure, and repairs of sub-standard quality on components; other causes.
