In 95% of accidents investigated personally, it was proved that origin of accident was due invariably to mechanical defect and that "pilot's error" occurred after this fault had set in; most vulnerable type and that most liable to accident is multi-engined airplane; need for simplifying job of pilot emphasized; suggestion made for composition of flight crews and responsibilities of each man.
