The mid-air collision of Brazil civil aviation in 2006 is a typical of accident caused by human error. We try to find out all attribute factors of the accident by reading accident report issued by CENIPA, and then analyze them with human error analysis system of China civil aviation (CN-HFACS) which is improved from HFACS to fit China civil aviation status. The ultimate cause of the accident is: 1) the N600XL crew's lack of adequate preparation of the flight, unfamiliarity with the aircraft and its avionics, 2) operation violation of ATCOs, poor ATC and English language skills. The contributing cause is lack of adequate management of civil aviation administration. To improve civil aviation safety, human factor education and human error research must continue to be strengthened.
