One of the organizational causes of the Shuttle Columbia mishap found by the Columbia Accident Investigation Board (CAIB) Report was a "Broken Safety Culture". Since its release, NASA has strived to improve its Safety Culture In 2009, the Chief of the Office of Safety and Mission Assurance asked a member of the Air Force Safety Center and of the CAIB for assistance to develop a NASA Safety Culture Program. This presentation describes the journey that NASA personnel has taken developing a NASA Safety Culture Program to "repair" its Safety Culture.
