This study was funded by the National Eye Institute and was conducted by the Pediatric Eye Disease Investigator Group (PEDIG). The study had 2 phases. The primary objectives of each phase were as follows: (1) Spectacle Phase, which was to determine in previously untreated pure anisometropic patients the incidence of resolution of amblyopia and the time course of visual acuity improvement with spectacle correction alone; and (2) Randomized Trial, which tried to determine whether 5 weeks of patching treatment (2 hours of patching per day of the sound eye combined with at least one concurrent hour of near activities) compared with a control group (using spectacle correction only) improves visual acuity in patients with amblyopia. At enrollment, patients who were prescribed spectacles for the first time, required a change in prescription, or had not worn optimal spectacles for at least 16 weeks entered the pre-randomization spectacle phase of the study. Visual acuity in each eye was measured every 5 weeks until the amblyopic eye stopped improving or amblyopia resolved. If there remained at least 2 lines of interocular acuity difference, the patient entered the randomized trial. Patients who did not require spectacles at enrollment or who had worn optimal spectacles for at least 16 weeks skipped the spectacle phase and entered the randomized trial phase directly. In the randomized trial, patients were randomized either to 2 hours of daily patching with one hour of near visual activities or to spectacles alone (if needed). Visual acuity in each eye was measured every 5 weeks until the amblyopic eye stopped improving or amblyopia resolved.
