System failure data for several flight control programs that have been in service for years are examined in detail with respect to software faults. The results show that reliable software systems can be met with well-tested and well-maintained single software designs, avoiding the expense of multiple, independently developed software programs that has been proposed to eliminate the generic software fault in multiprocessor systems. After one million flight hours in the field, one system exhibits continued reliability growth, with 10000 hours mean-time-between failures for the software. The failures were all noncritical, and over 60% of them were traced to incorrect requirements.
