Ordnance system failure analysis presents several unique challenges. These challenges include stringent safety requirements, assessing system and component interactions to isolate the causes of failure, and a frequent lack of physical evidence. This paper explores a systematic methodology developed by Aerojet Ordnance Company to overcome these constraints and allow an intelligent assessment of the most likely causes of failure. Key analytical techniques include fault tree analysis, hardware analysis, film analysis, telemetry analysis, pedigree analysis, 'what's different' analysis, and a disciplined failure mode assessment technique based on the output of the above analyses. The technologies discussed in this paper have been successfully imployed to find the root causes of air dispensed munition, ammunition, warhead, and fuze failures. The techniques are also applicable to ordnance accident investigations.
