Abstract As operator errors are among the most avoidable errors, and can involve culpability, they are often the most vaguely reported of incidents, whether for professional courtesy or out of legal considerations. Hence, specific details are rather difficult to find. On the other hand, it should be recognised that operators, who often work long stressful hours, are the heroes who have nursed spacecraft back from a variety of system failures. As an example of a simple mistake, on 5 December 1984 controllers mistakenly uploaded the NOAA 7 satellite with the ephemeris for NOAA 6; the satellite lost attitude control, but was recovered a day later.1
