To help determine the optimum system safety effort, space programs and projects for NASA, DOD, and commercial groups were reviewed to determine what efforts were performed and why these efforts were accomplished. Projects included the Space Shuttle, Inertial Upper Stage, Payload Assist Module, deployable fixed payloads of the Shuttle, and Expendable Launch Vehicle Payloads. Based upon the review and the assessment of the results, recommendations for ESA implementation are included.
