Between May 31, 1989, and March 17, 1991, five fatal accidents occurred in the United States involving Piper Aircraft Corporation model PA-46 airplanes. Twelve persons died in the accidents and the five airplanes were destroyed. The National Transportation Safety Board investigations and analyses of the accidents disclosed that one occurred because the pilot entered a very strong thunderstorm, lost control of the airplane, and overstressed critical structural components, which separated in flight. The causes of the other four accidents involved probable failure to use pitot heat during flight in freezing instrument meteorological conditions, possible misuse of integrated flight guidance and control systems, loss of control, and in-flight airframe failures due to loads and stresses that substantially exceeded design limits.
