The National Transportation Safety Board determines that the probable cause of the accident was the failure of the rail attendant of train No. 172-171 to follow Metro-Dade procedures by operating the train with the ATP system bypassed and his failure because of inattention, distraction, or the effects of drugs, to monitor the track ahead of the train, perceive the standing train, and react in time to stop his train safely. Contributing to the cause of the accident were flawed transit system procedures which resulted in the testing of trains with known equipment defects on the same track with revenue passenger trains.
