The National Transportion Safety Board determined that the probable cause of the fires on board the Panamanian tank ship SHOUN VANGUARD and the U.S. tank barge HOLLYWOOD 3013 was the failure of Intercontinental Terminals Company (ITC) management personnel to remove from service a deteriorated cargo hose and the failure of the ITC management personnel to follow written company procedures and U.S. Coast Guard regulations which required a cargo transfer hose to be fully purged of propylene and disconnected from the terminal pipeline when propylene transfer operations were discontinued.
