The National Transportation Safety Board determines that the probable cause of the accident was the failure of the operator of train No. 6601 to comply with the signal aspects displayed and to monitor properly the track ahead and react in time to safety stop the train, and the failure of the Greater Cleveland Regional Transit Authority to enforce strict compliance with operating rules, to maintain its signal system, to adopt unambiguous operating rules, and to monitor adequately the performance of its train operators, thereby creating a permissive block operation. Contributing to the accident was the failure of the GCRTA to prevent vegetation from blocking visibility in areas of critical sight distance.
