This summary was prepared to help commanders at all levels, aviation safety officers, maintenance officers, aviators, and related aviation personnel to prevent future accidents and preserve combat resources through a review of OV-1 mishaps and their cause factors. The term 'mishap' as used in this summary, includes accidents, incidents, forced landings, and precautionary landings, as defined by paragraph 7, AR 385-40. Aircraft losses or damages which were the direct result of hostile action in RVN are not included. Information presented in this summary was taken from accident and other mishap reports submitted from all Army commands, as required by section IV, AR 385-40. Information on Modification Work Orders (MWO's) was extracted from Department of the Army Pamphlet No. 310-7, U. S. Army Equipment Index of Identification Work Orders, April 1968.
