The case of a mechanic who suffered fatal injuries when a forklift truck overturned was examined. On July 19, 1983, the victim was performing a load test, without he knowledge or presence of his supervisor and without employing normal safety procedures. The forklift, which was being modified to handle large loads, was parked with the rear wheels on an asphalt pavement and the front wheels on the berm while it was carrying a load of 5,430 pounds. The machine was rated to carry 5,550 pounds with the mast raised vertically. The driver raised the load and tilted the mast forward to test the counterweight stability of the truck when the truck tilted forward and rotated about the front wheel axles. The driver then lowered the load and jumped off when his truck continued to rotate. He was struck by the mast of the forklift when it fell on him while he was seeking refuge about 12 feet away. It was concluded that this incident occurred due to the violation of several precautions for forklift operation, for failure to adhere to Standard Operating Procedure for forklift testing, and for moving in the direction of the forklift fall when seeking safety.
