This report explains the failure of tank car UTLX 79211 and release of arsenic acid while in the Norfolk Southern Railway Company's railroad yard in Chatanooga, Tennessee, On June 6, 1994. Arsenic acid entered a storm drain system within the yard, and arsenic-contaminated water was discharged into Citico Creek and then into the Tennessee River. Because intake pipes for the city's municipal water supply were located in the Tennessee River near the mouth of the creek, actions were taken to ensure that the area's drinking water was safe. The safety issues discussed in the report are: (1) the configuration and construction of education pipe systems in tank cars; (2) the adequacy of quality control inspections to ensure that tank cars are constructed in accordance with designs and that modifications are performed correctly; (3) the evaluation, selection, and inspection of coatings or linings to protect tank cars from corrosive damage by the cargo transported; and (4) the adequacy of actions taken in response to the leaking tank car.
