About 2215 on August 31, 1988, an explosion in cargo tank 1 of the 711-foot-long Maltese tank vessel FIONA killed one person and blew off the top of the cargo tank. The safety issues discussed in the report include: the contamination of the No. 6 fuel oil in cargo tank 1 by the previous cargo; the proper blending of the No. 6 fuel oil at its loading point; a static electrical discharge in cargo tank 1 as a result of a steam leak in the cargo tank; ungrounded measuring devices used in cargo tanks with potential explosive vapors; the production of explosive vapors by Grade E cargoes in enclosed vessel cargo tanks; the use of installed inert gas systems on tank vessels; the use of closed measuring systems on tank vessels; the design and maintenance of the FIONA cargo tanks; the absence of written procedures for the FIONA crew regarding tank cleaning, the testing of cargo tanks for explosive vapors, the use of the vessel inert gas system, and the safety procedures to be observed during the sampling and measuring of cargoes; and the adequacy of U.S. Coast Guard, International Maritime Organization, International Safety Guide for Oil Tankers and Terminals, and industry safety procedures for the measuring and sampling of Grade E cargoes.
