For the purpose of identifying critical situations during helicopter missions, two studies have been performed. Firstly, an analysis of incident reports from the Swedish Army Aviation during a five year period and second, an empirical study in which helicopter pilots reported incidents with regard to causes and degree of risk. The results showed that errors were most common during procedures before start, tactical missions, and landing. The most common errors were procedural errors, faulty judgements of observations and shortcomings in the exterior supervision. The primary cause was the disturbance of the pilot. In the light of the findings, recommendations are given.
