The National Transportation Safety Board determines that the probable cause of the grounding of the PILGRIM BELLE at Sow and Pigs Reef, Vineyard Sound, Massachusetts, was the failure of the master to actively direct the navigation of his vessel, to plot a course to alert himself to the possible dangers along the vessel's route, and to take and plot navigation fixes to monitor the vessel's positions accurately. Contributing to the accident was the failure of the mate to warn the master that the vessel was approaching close to Sow and Pigs Reef.
