The document represents the outcome of a synthesis to review the current medical standard covering commericial drivers with diabetes mellitus. The Standard (49 C.F.R. section 391.41(b) (3)) permits qualification of individuals to drive a commercial motor vehicle (CMV) if the person has no established medical history or clinical diagnosis of diabetes mellitus currently requiring insulin for control. A literature review of accidents involving diabetic drivers was conducted along with a review of the current state laws regulating insulin-using CMV drivers. The effectiveness of current diabetes treatment was presented. A risk assessment was developed and applied to the information available concerning insulin-using drivers. Public comments submitted to the Federal Highway Administration (FHWA) in response to the Notice of Proposed Rulemaking on the qualification of drivers with insulin-treated diabetes for CMV operation were tallied (125) and summarized. The role of a medical advisory board in individual petitions, development of regulations and the establishment of monitoring procedures was discussed.
