The specific working conditions in the fleet and the comparatively small amount of therapeutic activities lead to a situation where, after several years of work on the vessels, the physicians lose their skills. This entails a significant turnover among them. The general medical and health-antiepidemic activities of a ship medic occur under a situation that is markedly different from on shore. All of this requires special training from him. However, up to the present, no medical school gives a course on marine medicine, and there is no opportunity to raise the skills of ship medical workers. The ship physicians and feldshers should be highly skilled specialists, since each of them is the only representative of medicine on the ship under conditions which exclude any possibility of constant consultation with colleagues, and requiring the taking of rapid and correct decisions for the sake of saving the life and protecting the health of the sailors. The alternation of certain periods of work on the ships and on shore in specialized medical institutions would help to raise the skills of the ship medical specialists.
