Safety is a strategic priority in civil aviation industry. In order to assess operation risk of a civil aviation organization, a safety performance indicator system should be established to reflect the actual safety status and expose safety management and safety foundation issues. Firstly, a seven steps approach is presented to establish safety performance indicator system. All these safety performance indicators can be categorized as occurrence type, foundation type, management type, and operation type. Secondly, weights for different types of safety performance indicators are assigned based on Heinrich’s Law in conjunction with the importance and complexity of safety management process and safety foundation. Thirdly, a multi-level risk assessment model is proposed to assess operation risk of a civil aviation organization, departments and key posts based on safety performance indicator system. Finally, a case study is demonstrated to verify the efficiency of the proposed multi- level risk assessment model.
