Due to the construction and operation of the Three Gorges Dam, the navigational conditions upstream of the Three Gorges Dam have changed considerably. The drastic change of water level upstream of the dam has caused great impact on the navigation safety of ships in this area. Ship accidents occur frequently, especially during the discharging period of the dam. A risk evolution model combining the HFACS model and information flow theory is proposed to analyze ship navigation safety in the fluctuating backwater zone. The proposed model introduces risk from the information flow perspective in three layers, which are macro-, meso-, and micro-levels. At each level, three characteristics of information flow, i.e., correctness, timeliness, and completeness, are considered as potential causal factors of navigation safety system risk. To demonstrate the feasibility of the model, the sinking of the cargo ship “EYiDuHuoShuangYa”, which caused serious consequences and attracted widespread social concern, was selected as a case study in this study. The results of the study can be used by maritime stakeholders to adopt countermeasures for ship navigation risk mitigation.
