The invention relates to a rail transit standby communication and positioning integrated system, method and device and a medium. The system comprises a ground base station and a vehicle-mounted label. The ground base station is connected with the train control center through a rail transit existing network, the vehicle-mounted label is installed on a train and connected with the vehicle-mounted computer through a serial port or a network port, and the ground base station and the vehicle-mounted label transmit through UWB, so that interaction of a positioning frame and a communication frame is achieved. Compared with the prior art, the system has the advantages of integrating communication and positioning functions, enhancing the standby capability, continuously and accurately positioning the train position, saving the cost, being safe and reliable and the like.
