The embodiment of the invention discloses a method and device for predicting road accidents in a tunnel and a medium, and relates to the technical field of traffic safety, and the method comprises the steps: setting a plurality of monitoring points in a to-be-predicted tunnel in advance, and obtaining the monitoring data of each monitoring point; dividing the road image of the monitored road section of each monitoring point, and generating a lane area image corresponding to the current lane; identifying vehicles in the lane area image corresponding to the current lane to obtain vehicle parameters of the current lane; according to the number of the vehicles in the current lane and the distance between the vehicles, the accident probability between the vehicles in the current lane is determined; according to the vehicle operation parameters, the environment data and the road surface type of the tunnel to be predicted, determining the road surface adhesion coefficient of each monitored road section so as to determine the single vehicle accident probability of each monitored road section in the tunnel to be predicted; and generating a road accident prediction result in the to-be-predicted tunnel according to the accident probability between the vehicles in the current lane and the single vehicle accident probability of each monitored road section in the to-be-predicted tunnel.
