For commercial transport aircraft, human safety is aspect of reliability of first importance, defined as probability of flight completion without fatality or injury; second in importance are trip completion and schedule reliability; review of various approaches to designing safe aircraft; reliability analysis and philosophy of safety design as applied by Douglas Aircraft Co is summarized; use of simulators to test various cockpit arrangements to reduce frequency of design-induced pilot error accidents.
