Consideration of 2 primary factors is proposed to reduce human error in missile systems; proper design of equipment, operator's familiarity with complex missile equipment, and adequate data-feedback system; ultimate objective of discussion and proposals is to arrive at means of increasing effectiveness of missile systems through reduction in incidence of human-initiated failures.
