Obstructive sleep apnea (OSA) is a common condition that is associated with increased insulin resistance and β‐cell dysfunction; hence glycemic abnormalities and type 2 diabetes (T2D) are common in patients with OSA. In patients with T2D, OSA is associated with worsening glycemic control and diabetes‐related microvascular complications. In addition to T2D, OSA is associated with increased risk of road traffic accidents, hypertension and cardiovascular disease, and increased mortality. The mechanisms underlying the relationship between OSA and its comorbidities are complex and include intermittent hypoxia, changes to sleep architecture, endocrine and metabolic changes, increased oxidative stress, and sympathetic activation amongst others. Continuous positive airway pressure (CPAP) ventilation has been shown to reverse many of the molecular consequences of OSA and is associated with beneficial effect on OSA comorbidities. However, the impact of CPAP in patients with T2D on glycemic control and diabetes‐related complications is not clear and requires further research.
