Aeromedical transportation may be undertaken by aeroplane (fixed wing) or helicopter (rotary wing) aircraft. This distinction is useful, as each broad type of aircraft has its own set of advantages and challenges in terms of the aeromedical working environment. Air transfers are usually undertaken for one of the following reasons: to shorten the time taken to access an expert team or to access definitive care, to minimise the duration of time that an unstable patient is outside a hospital environment, or when consumables (e.g. oxygen) make road transport inappropriate. This chapter explains the benefits and disadvantages of aeromedical transfer and the importance of the structured approach to aviation transfer medicine. It also considers the unique safety issues encountered in aeromedical transfers. Helicopter landings need to be undertaken under controlled conditions and safety is a paramount consideration. Understanding some of the problems of the international repatriation of patients may help receiving hospitals.
