Risk is generally defined as a combination of the consequence's gravity and the occurrence probability of an undesirable event. In Human-Machine Systems (HMS), these undesirable events can be generated by technical components and/or human operators. That's why; risk analysis in HMS has to take into account on one hand, intentional and unintentional human failures, their consequences, and occurrence probabilities and on the other hand, technical failures, their consequences, and occurrence probabilities in order to improve both the risk analysis and the safety of the design. This paper presents a new complete Fault Tree method that integrates the technical failures and the human errors consequences and occurrence probabilities. It is especially meant for urban guided transportation systems.
